Heather’s other passion is supporting young adults with ADHD. She splits her time between working at Davis Occupational Therapy and also supporting the mental health needs of students at one or our fantastic Ontario Universities.
She sees first hand the impact that ADHD has on a first year student’s ability to establish a routine, manage deadlines, advocate for accommodations and the emotional impact of living away from home for the first time.
We are so pleased to have Heather facilitating this workshop series, tailored to get in front of the youth and their parents before they begin their first semester. Knowledge and connection are so powerful and will help in making this exciting life transition less bumpy!
This series has been developed by Heather to support both the youth AND the parent/caregiver. Each have their unique needs, questions, concerns and hopes. Heather has created a space for both groups to meet with others, create connections and support one another.
Heather Cholack, MSW, RSW
Heather has worked in a variety of settings, with people utilizing trauma informed, strengths based and client centred approaches. She creates a safe, inclusive, identity affirming space for people, bringing anti-oppressive awareness to herself and her work. She is passionate about being present and hearing people’s stories. Heather uses a combination of DBT, ACT, CBT, Mindfulness and EFT modalities.
Services & Fees
60 minutes Virtual session | $150
60 minutes In Person session | $165
Mileage charges for locations that are >10km from the therapist’s home destination
Location can vary from a clinic setting to a nature spot
Report Writing | starting at $150/hour
Fee Determination
Professional fees are set in accordance with the recommended fee range for occupational therapists and social workers in private practice, established by the Ontario Society of Occupational Therapists and the The Ontario College of Social Workers and Social Service Workers. Services are HST exempt under Canada Revenue Legislation. Davis Occupational Therapy does not provide services through MVA benefits, i.e. OCF18. All services are cash based, fee for service.
Payment Terms
Payment are processed each Monday by our CFO. The credit card on file will be charged unless you indicate you would like to pay be Etransfer.
Cancellation Policy
If you are unable to keep the appointment you have reserved, please call with more than 24 hours notice to avoid being charged. Appointments canceled with less than 24 hours notice will be charged the full rate of the service booked.
